目的

探讨血液乙型肝炎表面抗原(HBsAg)阳性孕妇早期流产的绒毛组织中乙型肝炎病毒(hepatitis B virus, HBV)标记物检测及高危因素与载脂蛋白H(apolipoprotein H, Apoh)表达的关系。

方法

选取2017年1月至2018年5月在深圳市第三人民医院行早期流产的41例血液HBsAg阳性孕妇的绒毛组织和血液样本,应用荧光定量qRT-PCR方法测定绒毛组织中HBV DNA含量,并检测孕妇入院时血液HBV DNA及乙型肝炎标志物的表达水平。采用免疫组化方法检测绒毛组织中神经胶质细胞缺失蛋白1、HBsAg和Apoh的表达情况。

结果

41例血液HBsAg阳性孕妇的绒毛组织HBV DNA的阳性率为29.27%(12/41),显著低于孕妇血液中HBV DNA的阳性率(53.66%, 22/41),差异具有统计学意义( χ2=5.025, P=0.043);绒毛组织中,HBsAg和Apoh阳性率分别为17.07%(7/41)和19.51%(8/41);Apoh在绒毛组织HBsAg阳性组表达率71.4%(5/7),高于HBsAg阴性组(8.8%,3/34),差异有统计学意义( χ2=14.487,P=0.000),Apoh在绒毛组织HBsAg阳性组表达率71.4%(5/7),高于HBV DNA阳性组25%(3/12),差异有统计学意义( χ2=3.909,P=0.048)。

结论

血液HBsAg阳性孕妇早期流产绒毛组织可检测出HBV DNA和出现HBsAg表达;绒毛组织中Apoh表达在绒毛HBsAg阳性者高于绒毛HBV DNA阳性者。

Objective

To investigate the relationship between the expression of Apolipoprotein H (Apoh) in the first trimester villi tissues of pregnant women with HBsAg positive and hepatitis B virus (HBV) intrauterine infection.

Methods

Villi tissues and blood sample of 41 HBsAg positive pregnant women during the first trimester were selected from January 2017 to May 2018. HBV DNA load in villi tissues were quantified by quantitative reverse transcription polymerase chain reaction (qRT-PCR). HBV DNA and hepatitis B markers in the blood of pregnant women were tested at admission, glial cells missing 1. HBsAg and apolipoprotein H in villi tissues were detected by immunohistochemical method.

Results

The positive rate of HBV DNA in villi tissues was 29.27% (12/41), significantly lower than 53.66% (22/41) in the blood ( χ2=5.025, P=0.043). The positive rates of HBsAg and Apoh in villi tissues was 17.07% (7/41) and 19.51% (8/41), respectively. The expression of apolipoprotein H in villi tissue HBsAg positive group was71.4%(5/7), higher than HBsAg negative group(8.8%, 3/34) ( χ2=14.487, P=0.000). The expression of apolipoprotein H in villi tissue HBsAg positive group was also higher than HBV DNA positive group 25%(3/12) ( χ2=3.909, P=0.048)

Conclusions

HBV DNA and HBsAg can be detected in villi tissues during the first trimester in HBsAg positive pregnant women. The expression of apolipoprotein H may was closely related to villi tissue HBsAg positive.
