重度子痫前期并发急性左心衰的诊断和处理

重度子痫前期并发急性左心衰是妊娠期高血压疾病的严重并发症，严重威胁孕产妇生命安全。产科医生应加强对子痫前期患者的管理，积极预防、早期识别和早期诊断急性左心衰，采用合理方案救治。对子痫前期患者提供长期健康指导计划，可改善预后和提升生活质量。

关键词: 子痫前期, 心力衰竭, 诊断, 处理

Preeclampsia complicated with acute left heart failure is the serious complication of hypertensive diseases during pregnancy, which seriously threatens the life safety of pregnant women. Obstetricians should strengthen the prevention, early diagnosis and correct management of patients with acute left heart failure from preeclampsia. Providing long-term health plan may improve the prognosis and quality of life.

监测 (1)经皮SpO2
  (2)血气分析：pH值和PaCO2 (包括乳酸)
措施 (1)考虑急性左心衰均应立即给氧
  (2)无创正压给氧　呼吸>25次/min, SpO2<90%
  (3)气管插管给氧　PaO2<60 mmHg [8.0 kPa]; PaCO2>50 mmHg [6.65 kPa]; pH<7.35
