患者33岁，孕2产0，因"孕27⁺¹周，阴道出血4 h"于2022年4月30日10时入住当地县医院。患者入院前4 h因活动后突发阴道流血，色鲜红，略多于月经量，伴有腹痛。入院查体：身高150 cm，体重48.5 kg，阴道流血，色鲜红，宫口松，可容一指，腹壁可触及宫缩，不规则。予硫酸镁静脉滴注，诉腹痛缓解，阴道出血停止；当晚20时许宫缩再次增加，伴再次阴道出血。阴道检查：宫口已开全，与家属沟通后放弃保胎，送产房分娩。患者既往月经周期规则，2年前有一次早孕人工流产史，无分娩及手术史；本次妊娠早孕期因阴道出血曾使用地屈孕酮保胎治疗；县医院定期产检，中孕期因血糖高予以胰岛素治疗，血糖控制佳。
