The problem of blood pressure reduction in women with preeclampsia

Abstract

Abstract:

Preeclampsia is one of the most important reasons for the poor prognosis of pregnant women and perinatal infants. Antihypertensive therapy plays an key role in the management of preeclampsia and has been widely studied nowadays. This review was written based on the latest clinical guidelines for hypertensive disorders of pregnancy and our own experiences to discuss the problem of blood pressure reduction in women with preeclampsia.

降压药物	剂量	评价	起效时间
拉贝洛尔	起始10~20 mg，静脉注射，随后每10~30分钟给予20~80 mg，最大剂量可至300 mg；或持续静脉泵入，1~2 mg/min	心动过速少见，不良反应较少。禁忌：哮喘、以往存在心肌病、心功能失代偿、传导阻滞、心动过缓等	1~2 min
肼屈嗪	5 mg静脉或肌内注射，随后每20~40分钟给予5~10 mg，最大剂量可至20 mg；或持续静脉泵入，0.5~10 mg/h	药物过量引起母亲低血压、头疼以及胎儿心律失常。不良反应较其他药物常见	10~20 min
硝苯地平	10~20 mg口服，20 min可重复一次；随后每2~6小时口服10~20 mg，每日最大剂量为180 mg	不良反应：反射性心动过速、头痛	5~10 min
