Anticoagulation management strategies during pregnancy with mechanical prosthetic heart valve

Abstract:

The anticoagulation management of women with mechanical prosthetic heart valves during pregnancy is challenging and controversial. The thrombogenicity of mechanical prosthetic heart valve (MPHV), the hypercoagulable state and the limitations of current anticoagulation strategies lead to the high risk of the patient. There is no perfect anticoagulation regimen for this kind of patient. There are some risks and benefits of each approach for both mother and fetus. This review evaluates the existing data about the safety profile of various anticoagulation strategies and the potential benefits and limitations of each approach for the mother and fetus separately; and discuss the recommendations about the management of mechanical valves in pregnant women.
